Enrollment & Registration

       It is requested that all parents considering enrollment receive a tour of the facility and review the center’s general procedures and daily activities prior to attending. Children are encouraged to visit with the parents.

      The following forms must be completed for each participant and on file prior to his/her first day of attendance:

•        Registration form

•          Emergency Medical form

•          Proof of current immunizations

•          Discipline Policy

•          Safe Arrival/Departure Procedures

•          Statement of Permission

•          Payment Agreement

•          Statement of Receipt of Materials

•          Parent Questionnaire

•          Teacher Questionnaire

     Accurate information is vital to helping us provide the best possible care for your child. It is the parents’ responsibility to inform the Academy office immediately of any changes regarding personal information. All records are strictly confidential and the property of Triad Baptist Christian Academy.

ACADEMY 

     A curriculum and/or activity fee will be required of each student. These fees are renewable each August, but will not be collected more than once per physical year. These fees are non-refundable.

AFTER SCHOOL  PARTICIPANTS

     Weekly tuition includes care  during scheduled school closings according to the Winston Salem/Forsyth County calendar. In addition, inclement weather days (not to exceed 8)  are included. Fees for field trips and special activities on these days will be charged to each participant’s account.

SUMMER CAMP

          Generally there are 9 – 12 weeks of summer camp scheduled to begin and end in accordance with the Forsyth County school schedule.

A Summer Camp registration fee must be received by non after school participants.   Weekly tuition payments include two snacks and a hot lunch. On occasion participants will be asked to bring their own lunch due to field trip arrangements. Fees for field trips and special activities on these days will be charged to each participant’s account.

Upon enrollment, time, space, staff and provisions are reserved for your child. Days missed will not be deducted from your payment. Payment in the amount of  ½  the weekly tuition will be required for weeks pre-registered for but not attended .
